Joint Commission Provision of Care, Treatment and Services standard address the composition of the interdisciplinary team of a Primary Care Medical Home. Per the standards the organization identifies who is on the team; the team must include a doctor of medicine or doctor of osteopathy. The interdisciplinary team, in coordination with the patient, develops the patient’s treatment plan and works with the patient to reach planned outcomes.
